Survey it self is a wrong approach. People won't do right thing without any reward. If main focus of a restaurant is on growth and addressing the problems the customer should be offered some kind of incentive like a discount coupon or any free dish like sweets on every reasonable mistakes and problems they can point at. The survey is absolutely useless if all they get is complements.
