There could be a series of answers here, but most importantly; - not focusing enough on team/people/culture since Day 1 (we've done this) - extremely revenue/traction/user growth focused without clarity of bottomlines & sustainability - founders not learning finance/accounts from day 1 - outsourcing key channels of revenue to 3rd parties rather than learning/upskilling & doing themselves.
