Broadly Corruption, low per capita income and different priorities. Healthcare is a luxury in a country where the talking about the problems of underprivileged is a bait for vote.

Because doctors spent huge time and money to reach at that level and dont want to work in tier 3 and below towns, and giving a space for underspecialised doctor and quacks to prescribe, treat them
