The Federal Aviation Administration (FAA) has accepted SpaceX's "root causes and 17 corrective actions" related to the investigation of the second Starship launch in November. However, before flying the Starship again, SpaceX must implement the corrective actions and apply for a modified launch license, which is currently under evaluation. The investigation showed that the likely cause of the explosion during the launch was a filter blockage in one of the Raptor engines. SpaceX is already incorporating hardware changes and design upgrades for the next launch, aiming for a fully reusable launch system capable of various missions.
