Ratio of doctors to number of people might be the biggest problem. Also, if a patient visits a doctor, doctor can treat him in few minutes and can go to the next one, but if it is reversed, the doctors might spend their precious time on travelling. Anyway, i think the ambulances having the primary tools to treat the emergency patients is enough, except the cases like heart attack like emergencies. But it's better if the ratio increases, anyone can think about doing that.
